    New online transcript service offered through a partnership with Credentials Solutions.

            To order a transcript of Continuing Education history (non-credit):

            ALL requests must include your full legal name; date of birth and/or your Campus Wide ID number.

            Include information on where you would like the transcript to be mailed. Signature required if mailing to an address not on file.

            Continuing Education transcript processing can take up to two to five business days after all class rolls
            are received from instructors.

    The Admissions and Records Office provides these forms as a convenience to our prospective or registered students in order to expedite the admissions or registration process.

     (*)Most of these forms will be processed within one to two working days, except for the ones marked with an asterisk.

    Forms that require documentation will not be processed until the proper documentation is submitted. Students are responsible for submitting forms and/or documentation before any published deadline.
