The Student and Enrollment Services Office provides the following forms for admissions, registration, residency, and transcript request to students for your convenience. These forms require the student's signature. Please print, complete, and return to any Student and Enrollment Services Office on a campus near you.

Online Transcript Request From Collin College

 

To order a transcript of Continuing Education history (non-credit):

  1. Fax request to 972.985.3723
  2. Email request to CEHELP@collin.edu
  3. Call 972.985.3711

 

All requests must include your full legal name; date of birth and/or your Campus Wide ID number. Include information on where you would like the transcript to be mailed. Signature required if mailing to an address not on file. Continuing Education transcript processing can take up to two to five business days after all class rolls are received from instructors.

The following services are offered, free of charge, to current and former students:

  • Release of Student Records
  • Verification of Enrollment
  • National Student Clearinghouse Student Self-Service Enrollment Verification (online)
  • Change of Address /Correct or Add Social Security Number

*Most of these forms will be processed within one to two working days, except for the ones marked with an asterisk. Forms that require documentation will not be processed until the proper documentation is submitted. Students are responsible for submitting forms and/or documentation before any published deadline.
